Instructions for Using Transplant Medication List Software

Look at the tabs at the bottom of the screen.  They are:

 

·           Patient Guide:  This is the page you will print.  Don’t type or change this page!

·           Provider Data Entry Page:  This is where you will enter your data.  You can type on this page.

·           Tech use only 1:  This contains the code that makes this all work.  Don’t type or change this page!

·           Tech use only 2:  This contains the code that makes this all work.  Don’t type or change this page!

 

Now click on the tab for Provider Data Entry Page.  You are going to fill in the yellow boxes.

Enter the patient’s name in the yellow boxes at the top.  Also enter the phone number for your clinic – the number patients should call if they have questions about their medications.

 

Under the medication sections, use the drop-down menus to select the drug.  Then put the dose in the yellow box under the appropriate time.  For example, you could choose tacrolimus with the drop-down box and then type “5 mg” in the box at 8:00 am and 8:00 pm.

 

You can change the dose times in the top yellow row if you like.  Just type in the new times.  Notice that the time will change for the whole column.

 

If a drug is not listed on the sheet, check the drop-down box at the bottom under “Other medications.”  If you still don’t find it, then type the drug name into one of the large yellow boxes under “Other medications.”  Then enter the doses under the appropriate times.

 

Now for the fun part.  Click on the tab labeled “Patient Guide.”  Notice that all the information you entered appears here, and some additional information has appeared under each drug.  Now print this page and it will come out on one nice sheet of paper.  (Don’t print the “Provider Data Entry Page” for the patient – it will not look as good and will not contain the extra information.)
