StoneSheet-24

24-hour urine collections for stone risk factors are useful, but difficult to interpret in pediatric patients.  The normal ranges in pediatrics are very different; some vary by age and some take into account weight or GFR.  This Excel spreadsheet takes the results from a 24-hour urine study and recalculates them according to age-specific pediatric normals. It also determines estimated (Schwarz formula) and calculated (UV/P) creatinine clearance.  You can compare the two to help evaluate the completeness of the 24-hour collection.  It prepares a printable one-page summary with all the information, graphs, and space for you to write or type your interpretation.  Citations for the reference ranges are provided.  This solution was presented as a poster at the American Medical Infomatics Association 2003 Annual Meeting.  This is designed to work with the Mission Pharmacal StoneRisk Profile and Quest’s #442 “Stone Risk Diagnostic Profile,” (which is the same and actually goes to Mission), but it should work with any 24-hour urine tests, as long as you pay close attention to the units.  You can also customize the printout with your hospital's logo.  This spreadsheet is not affiliated with or endorsed by any commercial laboratory service.

StoneSheet Spot

Less elaborate than the StoneSheet-24, this Excel spreadsheet lets you enter the results of random or “spot” urine tests for stone-related substances such as calcium, oxalate, uric acid and citrate.  You enter patient information and results, and the spreadsheet calculates the ratios and presents them graphically along with age-specific reference ranges.  Space for notes or interpretation, handwritten or typed, is provided.  All of this fits on a single 8-1/2 x 11 page, ideal for printing.  You can also customize this with your hospital's logo.

Lab Result "Report Card" Generator

This solution provides a one-page summary of monthly lab results for dialysis patients.  Large dialysis chains already have software to generate these reports; now it is available to small independent centers as well!  It shows the results, the reference ranges, and a comment on the results and what they mean to the patient.  You can edit the reference ranges to agree with the values used at your center.

Kt/V Calculator

This solution calculates Kt/V (and URR) for hemodialysis using the Daguirdas II formula.  It creates a printable one-page summary, comparing prescribed to delivered.  Values below K/DOQI standards are flagged in red.  This may be helpful for prescribing dialysis as well, as you can try different combinations of time, blood flow, etc. and compare the predicted Kt/V.  This is quick and simple solution.  Be sure also to look at Asher Schachter's kt-v.neta very sophisticated online solution.

Transplant Medication List

This solution prepares a printable, one-page medication list for your transplant patients.  You can quickly select the medications from the drop-down lists, or enter other unlisted medications.  It includes the drugs, the doses, a graphic chart of when to take them, and special information for each drug.  Keep a copy online in your clinic, and you will always have access to your patients' most recent medication list. Improve adherence by printing out a current list and giving it to your patients at each visit.

Ambulatory Blood Pressure Monitoring Report Template

This fill-in-the-blank template creates a written report and interpretation for a 24-hour ambulatory blood pressure study.  It calculates BSA, % sleep decline automatically and flags most results as normal, intermediate or abnormal.  It is fully customizable.

This is a pocket version of the StoneSheet-24 listed above.  Due to the limitations of Pocket Excel, it is much less elaborate, and does not produce a nice printout.  It may be helpful for a quick evaluation of results.  Runs on Pocket Excel.

A quick creatinine clearance calculator.  Uses the Schwartz formula, and will also calculate a formal creatinine clearance from a 24-hour urine collection.  Converts inches to centimeters, too, since Schwarz uses centimeters.  Runs on Pocket Excel.
